A. Duty to Provide Sales Receipts. Wellness Advocates must provide the customer with a completed retail sales receipt at the time of the sale.
1. The retail sales receipt should be completed to include the following information:
a. The Wellness Advocates’ full contact information, business names and Value-Added Tax registration numbers;
b. Name and description of goods;
c. Date on which the transaction/s took place;
d. Unit price of goods purchased;
e. Quantity of goods purchased;
f. Total price of transactions; and
g. Full contact information, business names and addresses.
2. Wellness Advocates should keep copies of all retail sales receipts on file for at least six years or such longer period as may be required by law. The amount of sales tax collected must be recorded on the retail sales receipt form.
